Patients begin their journey with PainCare Connect by first completing a patient-intake survey. Here, patients provide the care team with a thorough and detailed overview of their medical history, the origins of their pain, and any other relevant details that will be crucial for providers to be aware of. The initial consultation will be conducted by one of PainCare Connects physician assistants/nurse practitioners. Thereafter, a physician will consult with the patient and conduct a treatment plan for the patient and the care team. Following this appointment, initial consultations will also be conducted by a physiotherapist, psychotherapist, and nutritionist.
-
After the first two weeks of initial consultations, patients will receive follow-up appointments with the care team. Patients will work closely with the care team for the remainder of their treatment journey.
-
Goal setting is an important feature of the pain care journey for patients. Our unique software is patient-provider centred, meaning that patients, and the care team can interact with the software and incorporate goals that are most important to them.
-
PainCare Connect's partnership with Telus Health's Collaborative Health Record system allows providers to work in unison, irrespective of where they are located. Say goodbye to information siloes. We make it easy for providers to work as a team, to access information when needed, and to track patient reported outcome measures.
-
PainCare Connect is a data-driven program. We want providers and patients to understand what's working, and what's not working. We want patients to understand what's driving their pain. Through the course of the program, providers and patients will have constant access to data driven metrics that will track pain scores and patient progress.
-
As alluded to, goal setting is an integral component of PainCare Connect's treatment journey for those in chronic pain. Complementary to this, is a comprehensive return to work program. Giving autonomy back to those most in need, our case managers will work alongside patients, systematically helping them return to work.
The Program
What the science says
Several studies have evaluated the effectiveness of telemedicine for chronic pain, compared to traditional in-person care.
Results have shown that:
Patients receiving telemedicine pain care services were twice as likely to report a 30% improvement in pain scores, when compared to those receiving care in-person
Patients receiving telemedicine pain care services reported an improvement in depressive symptoms (P<.001) than those receiving in-person care, and fewer days of disability (P=.34)
Patients receiving virtual care accrued $450 in cost savings when compared to those receiving in-person care
87% of respondents indicated a preference for virtual health over traditional in-person care
